I love St David's - the city and the cathedral. If you put a marble on the floor in one corner if the cathedral I think it would roll diagonally to  the other corner!  :D

I don't think I can include a hot air balloon (sadly) as I don't think I have enough sky ...

unless I make it very wee as in, far off in the distance?

What sort of ideas are you looking for? You've had lots of ideas.

Shop. School. Car park. Bonfire. Holiday jet flying by. Sun. Clouds. Birds. People.

Oooh, yes.  A nice little village school like this one at which I used to teach in Devon.  It would need to be close to the church as village schools are often church schools.

I have some llama fabric.  A field of llamas would be great but I think you'd have a problem with scale.

It doesn't have a pub or a Post Office. Maybe a little bridge over a river and if you're feeling particularly ambitious, a mill with a wheel beside aforementioned river.
It hasn't a chippie or a butcher's shop or a greengrocer. A window cleaner in a van and a pharmacy might be useful. A police station, a train station, some road signs to interesting places. A town hall. Has someone said cuddly toy already???  :)

I really love this quilt, Imi: tempted to have a go at one! I too thought a school or a village hall would be good. Maybe a post box if not too small.
